Kyle Fundytus, a 16-year-old "Southside Athletic Club Don Wheaton Midget AA centre was hurt during the third period of a game against the Maple Leaf Athletic Club Raylin MFG." Apparently, he was struck in the neck with the puck, and it was in a fluky enough spot that he had to be rushed to the hospital in critical condition. The league later confirmed his death.
